- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor patient health by checking vital signs and observing for any changes in condition or behavior.
- Prepare nutritious meals tailored to individual dietary needs and preferences.
- Administer medications accurately following care plans and HIPAA regulations to protect patient privacy.
- Maintain a clean and safe environment through regular cleaning, laundry, and sanitation tasks.
- Support residents with memory care needs such as Alzheimer's or dementia by implementing specialized techniques.
- Use assistive devices like Hoyer lifts or resident's to ensure safe transfers and mobility assistance.
- Document care activities meticulously in compliance with healthcare standards and regulations.
- Collaborate with healthcare professionals to develop and update personalized care plans.
- Provide emotional support and companionship to promote mental well-being and social engagement.
Skills
- Proven experience in home & community care, assisted living, nursing homes, or long-term care environments.
- Knowledge of patient observation techniques, vital signs monitoring, and medication administration.
- Familiarity with specialized care such as Alzheimer's/dementia/memory care and working with individuals with developmental disabilities or autism.
- Certifications in CPR, First Aid, HIPAA compliance, and hospice or home health care are highly desirable.
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary.
- Strong communication skills for working effectively with residents, families, and healthcare teams.
