Care Management Associate – Position Summary

As a Care Management Associate, you will support a collaborative clinical team dedicated to delivering high‑quality, cost‑effective care to our members. You’ll help ensure individuals receive the right services at the right time by coordinating care activities, streamlining workflows, and enhancing the overall member experience.
What You’ll Do
- Engage with members through proactive outreach to support seamless care coordination.
- Review and process prior authorization requests to keep care moving efficiently.
- Triage and manage incoming Care Team tasks with accuracy and urgency.
- Identify the reason for admission and apply appropriate assessment tools to guide care decisions.
- Screen members using established business rules to determine service needs and make referrals.
- Track cases for discharge planning and flag outliers for further clinical review.
- Connect members to supportive programs such as Case Management or Disease Management.
- Maintain up‑to‑date and accurate member information using internal platforms (e.g., Dynamo).
- Assist in developing and implementing care plans under the direction of clinical staff.
- Encourage in‑network provider use and promote cost‑effective care options.
- Coordinate communication among providers, internal teams, and members to keep care aligned.
- Conduct non‑clinical research to support case management activities.
- Provide administrative support including call handling, issue resolution, and documentation.
- Ensure compliance with regulatory requirements and organizational policies.
- Uphold strict confidentiality standards to protect member information.
- Support claims research and contribute to continuous improvement of quality and case management processes.
- Thrive in a fast‑paced, dynamic environment while meeting performance expectations for accuracy, productivity, and service.
Required Qualifications:
Strong knowledge of medical terminology and multitasking abilities.
2–4 years of experience as a medical assistant or in a medical office setting.
Experience with call centers (inbound/outbound) and prior authorizations.
Comfortable working in a fast-paced environment.
Must reside in New Jersey or Pennsylvania

Pay Range
The typical pay range for this role is:
$18.50 - $38.82
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
