Job Description
Maxim Healthcare Services is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for an adult patient in their home in Berwick, PA.

Responsibilities
- Provide direct one-on-one nursing care for an adult patient in the home
- Monitor and assess the patient's condition and report changes as appropriate
- Administer prescribed medications, treatments, and therapies
- Follow physician orders and individualized care plans
- Assist with disease management and health maintenance
- Maintain accurate and timely nursing documentation
- Educate and support patients and family members as needed
- Ensure patient safety and promote optimal health outcomes
- Communicate effectively with the clinical team and caregivers
Qualifications
- Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing
- Current CPR/BLS certification
- Previous homecare experience preferred but not required
- Strong assessment and communication skills
- Ability to work independently in a home setting
- Reliable transportation
