This position is full-time, Monday - Friday. Employees are required to work any 8-hour shift schedule during the normal operating hours of 7:00 AM - 6:00 PM CST (9:30am-6:00pm CST current schedule) It may be necessary, given the business need, to work occasional overtime.
We offer 2 weeks of core training and then job shadowing. The hours during training will be 7:00 AM - 6:00 PM CST from Monday - Friday.

Primary Responsibilities:
- Provide care management services for members following discharge from behavioral health services
- Work directly with members, their families, and providers to ensure access to appropriate professional services and community resources to promote recovery and resiliency
- Provide viable referrals, and supports to ensure the members can remain safe and cared for in the community
- Assist medical and behavioral clinical staff with ensuring the member has the right care at the right time and accurately document that support in the clinical record
- Handle resolution / inquiries from members and / or providers to support members care needs

Required Qualifications:
- High School Diploma / GED or equivalent work experience
- Must be 18 years of age or older
- 2+ years of customer service experience
- Experience with Microsoft Word, Microsoft Excel, and Microsoft Outlook
- Ability to work any full-time, 8-hour shift schedule between the hours of 7:00 AM - 6:00 PM CST from Monday - Friday including the flexibility to work occasional overtime based on business need
Preferred Qualifications:
- Experience with working in the behavioral health field, with knowledge and understanding of behavioral health diagnosis and treatment options
- Experience with working in the clinical system: ICUE and LINX
- Experience with managing a caseload
- Experience with working within the health care Industry and with health care insurance
- Experience in working directly with members to assess and meet identified needs
Telecommuting Requirements:
- Ability to keep all company sensitive documents secure (if applicable)
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy
- Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Residents Only: The hourly range for this role is $16.54 to $32.55 per hour.
