Primary Responsibilities:
- Specialty Care Management for Medicare and Retirement Population in designated states
- Accepting referrals and working with facility staff to secure alternative placements for members in acute settings
- Reviewing of daily census to identify possible placement barriers
- Actively working to reduce length of stay for Medicare members awaiting discharge placement to alternate living
- Attending various clinical rounds formats to consult as needed on cases
- Creating and maintaining database of local/regional alternate living facilities contacted
- Tracking active cases, monitoring success, and identifying areas of workflow improvement
- Identifying ways to add value to treatment plans and consulting with facility staff and internal staff and medical directors on those ideas

Required Qualifications:
- Licensed Master's degree in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D., or an RN with 2+ years of experience in behavioral health
- Licenses must be active and unrestricted to practice at the independent level
- 2+ years of post-Masters experience in a related mental health environment
- Proficient Microsoft skills (Word, Excel, Outlook)
- Dedicated office space and access to high-speed internet
Preferred Qualifications:
- Experience working with Medicare population
- Experience with mental health and substance abuse
- Experience with case management
- Experience working in managed care

Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
