Overview: As part of our commitment to advancing care delivery and improving patient outcomes, we are enhancing our end-of-life case management program, Aetna Compassionate Care Program (ACCP) to include bereavement and grief support.
The ACCP Bereavement and Grief Counselor will be responsible for providing emotional, practical, informational, psychosocial, grief and bereavement support to members, their families and staff. This role ensures compassionate, non-medical care, helping members navigate the complex emotions of anticipatory grief, end of life or legacy projects in alignment with their values and wishes.
Location: Work at Home - Eastern Time Zone
Schedule: Monday through Friday, 8:00am - 5:00pm Eastern Time
Key Responsibilities:
- Explain stages and reactions associated with grief, helping members and families understand what they are experiencing pre and postmortem.
- Help members challenge negative or distorted thoughts related to their loss, such as guilt or self-blame.
- Provide anticipatory guidance to members and their loved ones who are in any phase of their end-of-life journey.
- Educate members on coping mechanisms to manage difficult emotions and navigate daily life with grief.
- Offer emotional support and a calming presence to members and families.
- Facilitate conversations about end-of-life wishes, advance directives, and legacy planning.
- Support rituals and memory-making for members and their loved ones.
- Locate resources for support in the member’s community.
- Provide guidance on practical matters such as vigil planning, comfort measures, and resources.
- Participate, as needed, in interdisciplinary rounds with Medical Directors, Nurse Practitioners, Social Workers, Pharmacists, Behavioral Health and Case Managers. Accepts referrals from the Inter Disciplinary Team and Case Management.
- Coordinate with hospice, palliative care teams, and other service providers as needed. Internal and external.
- Plans and facilitates support groups for our team, our members and their families.
- Maintain confidentiality and uphold ethical standards.
- Provides emotional support to internal staff and Palliative Care Team.
- Available to prepare and conduct internal in-service seminars on religious subjects/issues upon request.
Required Qualifications:
- Minimum of 3 years’ experience in hospice, palliative care, counseling, or related end of life fields is required.
- Strong communication, empathy, and active listening skills.
- Ability to remain calm and supportive in emotionally charged situations.
- Cultural sensitivity and respect for diverse backgrounds and beliefs.
- High comfort level with technology.
Preferred Qualifications:
- Grief counseling certification
Education:
- Master’s degree preferred in psychology or counseling.
- License Practical Counselor specializing in Bereavement or Grief. Willingness to obtain multi state licenses.

Pay Range
The typical pay range for this role is:
$29.10 - $62.32
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
