Job Summary and Qualifications
This position supports the mission ofHCAHealthONEof optimizingthe quality of life of all those served by expanding beyond the traditional focus on medical care to meet the needs of the whole person. TheLead Case Manager(LCM)isresponsible fordelivery of care coordination and delivery of quality patient care with cost effectiveutilizationof resources. This position supports collaboration and communication among the patient/family,physicianand the entire health care teamregardingthe patient’s plan ofcareand continued needs post-discharge. TheLCMwill communicate daily with the discharge planners, social work staff, and URRN’stodeterminethe plan of care and communicate this to thephysicians. Communication with other members of the interdisciplinary team will be ongoing todeterminethe post-acute care needs of the patient. The communicationwith post-acute vendors; LTAC, sub-Acute Rehab, SNF, HHC etc. will be critical to ensureappropriateandtimelyreferrals are initiated. TheLCMwill be the primary contact for the discharge planners and the social workers to discuss issues that arise with patients, families, and physicians. Avoidable days will be recorded as a team effort with UR nursing,DCPand SW staff. TheLCM’swillbe requiredto report on length of stay issues and barriers to discharge with administration on a weekly or as needed basis.
TheLCMshares responsibility for managing day-to-day staffschedule,assignmentsand caseloads to ensureappropriate hospital-widecoverage, whilemaintainingfront line social work roles and case load.Provides expertise and staff support for problem–solving complex cases and issues and consults with the Director/Manager/other resources as needed. Collaborates with Director/Manager andparticipatesin staff evaluation and performance activities including monitoring, coaching, and educating as needed. Participates in required Case Managementdepartmentdata collection and medical record reviews/audits. Develops and supervisesnew stafforientation plans/onboarding.Assists the Director/Managerwith developing and implementing new protocols andprocess.In addition, TheLCMwillmaintaineffective communication between members of the care team.

Licensure/Certification/Registration: Current licensure in the state of Colorado as a Registered NurseorSocial Worker(LSW or LCSW).Specialty certification in case management preferred.
Education:Bachelor’s Degree in NursingorMaster’s Degree in Social Workfrom an accredited schoolrequired.
Experience:1-3yearsrecent clinical experience in an acute care setting. 5-7yearsrecent clinical experience in an acute care setting preferred.
Special Qualifications:Must haveexcellentwritten, verbal, and interpersonal communication skills andableto work with highly diverse populations. Must be able to use computer applications such as Word and Excel and understand review and interpretation of data for process improvement strategies. Must be knowledgeable of StateandJoint Commission standards. Must haveabilityto perform multipletakessimultaneously and work inhighstress environment. Must be able to applycritical thinking and formulateviablesolutions to the issues presented.
