Your Role as an LPN
In this role, you will provide high-quality, patient-centered care in a home setting:
- Deliver skilled nursing care in accordance with physician orders and care plans
- Assess and monitor patient conditions, recognizing changes in physical or emotional status
- Collect, analyze, and document patient data to support care planning and coordination
- Assist in developing and implementing individualized care plans
- Administer medications and treatments as prescribed
- Educate patients and families to support safety, independence, and overall well-being
What We’re Looking For
- Active Licensed Practical Nurse (LPN) license in Massachusetts
- Current BLS/CPR certification
- Current TB test or chest X-ray
- Strong clinical skills and attention to detail
- Compassionate, patient-focused approach to care
- One year of nursing experience preferred (new graduates encouraged to apply)
