RN Field Assessor – Make a Difference Every Day

What You’ll Do
- Conduct face-to-face assessments with members to evaluate medical needs and overall wellness.
- Help members access vital services such as:
- Adult & Pediatric Medical Daycare
- Personal Care Assistance
- Nursing Facility Custodial Care
- Personal Preference Program
- MLTSS Program Enrollment
- Complete 20+ assessments per week (based on need) using a holistic approach that considers co-morbidities, multiple diagnoses, and functional limitations.
- Collaborate with your team and supervisor to overcome barriers and achieve goals.
- Use your interviewing skills to engage members and uncover their health needs.
- Mentor new team members once you’ve mastered your role.
- Operate at full capacity by month four with comprehensive training and support.

Requirements:
- Must reside in Passaic, Morris, Sussex or Warren Counties NJ - Case Load and Travel will be spread across Passaic, Morris, Sussex and Warren Counties
- Must possess reliable transportation and be willing and able to travel up to 85% of the time. Mileage is reimbursed per our company expense reimbursement policy
- Active, unrestricted RN license, New Jersey.
- 1 or more years clinical experience (hospital, home health, or ambulatory care).
- Comfortable working with diverse populations in various settings.
- Strong attention to detail, organizational skills, and ability to work independently.
- Computer proficiency and experience handling medical records.
- Successful completion of the NJ Choice Certification, including Minimum exam score of 80%, Completion of state training modules and Field mentoring component
- Ability to work Monday–Friday, 8:00 AM–5:00 PM EST.
Preferred Skills:
- Assessment, case management, or discharge planning experience.
- Managed care or crisis intervention background.
- Strong interviewing and communication skills.
- Proficiency with MS Office Suite and EMR systems.
Education
- Associate Degree in Nursing required; BSN preferred.

Pay Range
The typical pay range for this role is:
$66,575.00 - $142,576.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
