We are looking for an Advanced Clinical Pharmacist to be our in-house Diabetes Champion at St. Mark's Hospital.
This is a highly visible, dynamic role where you will bridge the gap between bedside patient care, clinical providers, and hospital leaders. If you love the science of pharmacotherapy just as much as you love seeing the "lightbulb moment" when a patient finally understands their care plan, this is the opportunity for you. You will be the clinical authority and primary advocate for our diabetic patient population, shaping how our hospital delivers care from the top down.
What You’ll Do
As our resident expert in diabetes management, your day-to-day will be a fulfilling mix of direct patient interaction and high-level strategic leadership:
- Empower Patients: Lead comprehensive, compassionate education sessions for patients facing a new-onset diabetes diagnosis, equipping them with the knowledge and confidence to manage their health post-discharge.
- Lead the Charge: Serve on the hospital's Diabetes Committee, working with a multidisciplinary team of providers, nurses, and staff toward clinical excellence.
- Shape the Standard of Care: Develop, review, and maintain hospital-wide policies and protocols related to glycemic control and diabetes management.
- Drive the Data: Track, analyze, and report on key quality metrics and clinical outcomes for our diabetic patients, using data to continuously elevate our standard of care.
- Collaborate & Consult: Act as the go-to resource for hospital staff and providers, fostering a team-oriented environment dedicated to optimal patient outcomes.
In addition to the required duties of a clinical pharmacist, in this advanced role:
- You will sustain the hospital formulary by minimizing non-formulary procurements, utilizing therapeutic interchanges, promoting rational drug therapy selection, and performing formulary reviews and presentations.
- You will attend and actively participate in daily multidisciplinary and bedside rounds.
- You will work with medical staff to adapt and promote rational drug therapy and make improvements identified by drug utilization evaluations.
- You will actively participate in pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
- You will participate and/or lead a clinical program committee at the hospital or division level. Examples include, but are not limited to; antimicrobial stewardship, anticoagulation, critical care, pediatrics, neonates, etc.
- You will support and participate in the goals of the organization’s quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.
- You will participate in the data collection of drug utilization evaluations, adverse drug reactions, and pharmacy quality initiatives and complete notification reports regarding medication errors and adverse drug reactions.
- You will lead the training and mentorship of pharmacists, pharmacy students, residents, technicians, nurses and physicians.
What qualifications you will need:
- Pharmacist
- (PHARM-CS) Pharmacist - Controlled Substance must be obtained within 90 days of employment start date
- 1 to 3 years of dedicated clinical experience managing diabetic patients
- Certified Diabetes Educator (CDE / CDCES) designation is highly preferred.
