Job Summary and Qualifications
The PPS Coordinator promotes high quality, patient-centered care by providing clinical support in the prompt, accurate completion of of the IRF Patient Assessment Instrument (IRF-PAI), to ensure accuracy in the scoring of functional status and reflection of the patient’s care needs. In collaboration with the care team, the PPS Coordinator also assists in the ongoing monitoring of identified benchmarks for the inpatient rehab population. The PPS Coordinator accesses confidential patient information and maintains confidentiality of all information.
What you will do in this role:
- Oversees and ensures the accuracy and completion of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) prior to electronic transmission within required time frames.
- Maintains a current knowledge of IRF-PAI software and provides expertise on this software within the inpatient rehab setting.
- Maintains updated and ongoing knowledge of the regulations concerning the IRF-PAI and its completion within the inpatient rehab setting.
- Thoroughly researches and reviews clinical documentation and makes recommendations to clinical supervisors in areas needing improvement to support and validate rehab criteria and documentation compliance.
- Collaborates with physicians and interdisciplinary team members to capture accurate assessment of patient, including assessing patient treatment plans to facilitate achievement of expected outcomes.
- Verifies appropriate CMG designation through chart review, staff interview, and collaboration with Rehab Medical Director.
- Participates in ongoing education to the staff to ensure prompt, accurate completion of the IRF-PAI. Audits IRF-PAI, clinical documentation, and billing information periodically to ensure accuracy and consistency.
- Works closely and collaboratively with all members of the clinical team to ensure compliance at all levels.
- Evaluates patterns of variances from standardized care plans, and recommends strategies to resolve individual and caseload issues.
- Interacts with patient, family, staff, and physicians in a professional manner at all times, and handles patient, visitor, or physician complaints promptly and professionally.
- Serves as a resource to department staff, students, volunteers, and other departments, and contributes to the teamwork essential to ensuring quality patient care.
- Promotes participation, cooperation, and teamwork. Demonstrates problem solving in resolving issues.
- Participates in internal chart reviews and external audit coordination as needed.
- Participates in hospital or departmental performance improvement activities, and seeks opportunities to improve department and inter-department processes.
