About Our CarePartners PACE Program:
The CarePartners PACE Program (Program of All-inclusive Care for the Elderly) offers a comprehensive, community-based care model for seniors with complex medical needs. Our center serves as both a health insurer and healthcare provider, delivering highly customized, integrated medical and social services designed to help participants avoid nursing home placement and remain safely in their own homes.
Our PACE program provides total, wrap-around care, including primary medical care, physical and occupational therapy, social work, prescription management, and home care. In addition, our newly expanded Adult Day Center offers a vibrant, social hub where participants enjoy recreational therapy, group exercises, and nutritious meals. The entire program operates under a dedicated interdisciplinary team that collaborates daily to manage every aspect of a participant's care plan, ensuring smooth coordination and ultimate peace of mind for seniors and their families.
What Sets Us Apart:
The CarePartners PACE Program combines the capabilities of an outpatient medical clinic with the compassionate, holistic environment of a community-focused senior center with the advanced clinical resources of Mission Health. Our team takes immense pride in delivering high-quality, patient-centered care that addresses not just physical health, but emotional and social well-being. Because we operate with an Interdisciplinary Team (IDT)—where physicians, nurses, therapists, and social workers all sit at the same table—you are never isolated in your care decisions. The unique PACE model allows for deep, meaningful connections with participants over time, shifting the focus from reactive treatment to true, preventative wellness.
