Job Summary and Qualifications
Organization Policies & Process Improvement Actively contributes to and works toward hospital wide improvement in meeting core measures, patient safety and service excellence goals. |
| Continuously evaluates work process and design; understands role in ensuring quality/performance improvement, productivity, and service delivery to meet stakeholder needs. |
| Performs duties in accordance with departmental policies and procedures, and recognized professional, quality, patient safety, environmental and infection control standards. |
| Demonstrates knowledge of the occurrence reporting system. Uses system to report potential patient safety issues. |
| Follows established guidelines for reporting a significant medical error or unanticipated outcome in the patients care which results in patient harm. |
| Identifies and communicates any patient, family and significant other education needs in collaboration with other disciplines, and participates in on-going medication education relevant to those needs. |
| Provides periodic medication related educational in-services to hospital staff and other caregivers. |
| Anticipates and responds quickly to patient needs as assigned within scope of practice. |
| Acknowledges patients appropriately. Introduces self and others. Explains purpose of visit/care and communicates anticipated duration to enhance patient experience. Uses key words at key times. |
| Demonstrates patient connection utilizing "sit, touch and talk" as appropriate. |
| Fosters positive first impression by ensuring all work areas are organized, clean and project a professional appearance. |
| Promotes a professional image and acts as an ambassador inside and outside the organization. Takes ownership and demonstrates service excellence principles in all interactions with the patient, physician and others. Ensures patient's safety, dignity and privacy at all times. |
| Conscientiously performs formulary substitutions as appropriate to assist the department in meeting drug cost budget. |
| Assists with cost containment measures related to high-cost and/or newly emergent pharmacotherapy that may adversely impact pharmaceutical spending. Identifies pharmacoeconomic opportunities, designs improvement plan, affects change, and monitors outcome. Submits and analyzes financial and operational reports. |
| Optimizes use of supplies, equipment and services, ensuring a high level of clinical care at the lowest cost. Actively seeks ways to control costs without compromising patient safety, quality of care, or the services delivered. |
| Actively participates in improving operational efficiency, effectiveness and achievement of established goals/performance metrics. |
