Responsibilities
- Deliver skilled nursing care in a home setting
- Administer medications, treatments, and physician-ordered interventions
- Monitor patient conditions and report changes to the care team
- Implement individualized care plans
- Complete accurate documentation and charting
- Collaborate with physicians, caregivers, and interdisciplinary healthcare teams
Required:
- Active Licensed Practical Nurse (LPN) license
- Current BLS/CPR certification
Preferred:
- 1+ year of nursing experience
- Experience with tracheostomy (trach), ventilator (vent), G-tube, enteral feeding, seizure management, or other skilled pediatric/homecare cases preferred but training available
