Job Summary and Qualifications
POSITION SUMMARY:
· This Clinical comprehensive role combines the technical expertise of a Mammographer with the crucial patient support and Clinical Coordination duties of a Breast Patient Navigator /High-Risk Breast Navigator, along with performing mammography. A breast patient navigator specializing in mammography and biopsies acts as a guide and advocate for patients needing breast imaging diagnostics, particularly when a potential abnormality requires further investigation through biopsy. High Risk Breast Navigator serves as a patient advocate for high-risk and breast imaging patients, from screening to diagnosis and beyond
· The clinical coordination duties involve coordinating patient care throughout diagnostic and treatment processes, scheduling procedures, and serving as a central point of communication among staff, physicians, and patients. The role also supports operational performance, assists with financial processes and inquiries, prepares cases for review, acts as a liaison, trains personnel, and maintains accurate records.
· This individual will be a key member of the multidisciplinary breast care team, providing direct patient care, guiding patients through complex diagnostic and treatment pathways, and ensuring efficient and high-quality operations within the breast center.
MAMMOGRAPHER
Key Responsibilities
· Perform routine and complex breast and imaging and examination procedures in mammography, ensuring high-quality images for accurate diagnosis.
· Ensure patient safety and comfort during imaging sessions, including proper positioning and compression techniques.
· Operate and maintain mammography equipment, performing quality control tests and troubleshooting as needed.
· Educate patients on the mammography process, answer questions, and provide guidance on breast health and self-examinations.
· Assist radiologists during procedures like breast biopsies.
· Maintain accurate patient records and imaging documentation.
· Maintain a second Mammography registry with the ARRT by annually accruing required ECE specific to Mammography education.
· Perform other duties as required or assigned.
BREAST PATIENT NAVIGATOR
Key responsibilities
· Guidance and Support: Provide emotional support and reassurance to patients and their families throughout the diagnostic process, understanding that a call-back for further testing or a biopsy recommendation can be anxiety-inducing.
· Appointment Coordination: Streamline scheduling of diagnostic mammograms, ultrasounds, biopsies, and follow-up appointments, minimizing wait times and ensuring timely care.
· Patient Education: Explain procedures like mammography and biopsies, potential results, treatment options, and relevant resources, ensuring patients understand their situation and can make informed decisions.
· Communication Facilitation: Serve as a liaison between the patient, their primary care physician, the radiologist, and other specialists, facilitating clear communication and ensuring all parties are informed.
· Biopsy Support: Follow up with patients after biopsies, addressing concerns, providing post-procedure care instructions, and discussing biopsy results and recommendations.
· Advocacy: Champion the patient's needs and concerns within the healthcare system, ensuring they receive appropriate and timely care.
· Documentation: Ensure comprehensive documentation of all patient interactions, biopsy results, recommendations, and communications within the electronic medical record.
HIGH RISK BREAST NAVIGATOR
Key responsibilities
· Serve as a patient advocate for high-risk and breast imaging patients, from screening to diagnosis and beyond.
· Identify high-risk patients through screening tools and review results with patients, educating them about their risk and the high-risk women's program (HRWP).
· Assist with scheduling appointments for genetic counseling, high-risk consults, and other related services.
· Provide education and resources to patients and families regarding breast health, treatment options, and available support services.
· Track and follow up with patients needing further diagnostic evaluation or biopsy.
· Facilitate communication between the patient, their family, physicians, and other healthcare providers involved in the patient's care.
· Report back to referring providers on high-risk screening results and management plans.
· Collaborate with the High-Risk management practitioner to maintain contact with high-risk patients annually to maximize adherence to care plans.
· Educate patients on understanding their risk, treatment options, and navigating the healthcare system.
· Identify and address barriers to care, providing referrals to support services as needed.
· Provide emotional support and counseling to patients and families throughout their breast health journey.
What qualifications you will need:
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
- (RN) Registered Nurse state of NH or valid compact state
- Associate Degree in Nursing
- 3-7 years related experience
- BLS
