Clinical Rehabilitation Specialist
We are currently looking for an ambitious Clinical Rehabilitation Specialist to join our team! A Clinical Rehab Specialist acts as a coordinator between internal/external referral sources and acute inpatient rehab operations. If you are dedicated to caring for the well-being of others, this could be your next opportunity.
Your duties and responsibilities may include:
- Establishing relationships with case managers, physicians, payers and other referral sources
- Ensure appropriate patient placement through clinical screenings and assessments in collaboration with both the hospital case management team and the Rehab Medical Director
- Work closely with the hospital case management team to ensure a responsive and smooth-flowing referral and admission process for the inpatient acute rehab program
Requirements:
- Graduate of an accredited school in clinical specialty
- Minimum 2 yrs. recent clinical care experience
- Licensed Professional in a clinical health care specialty (i.e., RN, Physical Therapist, Occupational Therapist, or Speech Therapist)
- Pass Functional Independence Measurement (FIM) within 90 days upon hire, then maintain current certification
- If RN, CRRN preferred and/or achieved within 2 years of hire
- BCLS certification (AHA)
- Clinical experience in the care and management of Inpatient Rehabilitation patients
- Experience as a clinical liaison coordinating admissions for Inpatient Rehabilitation patients
