Job Summary and Qualifications
GENERAL SUMMARY OF DUTIES:
The Population Health Navigator (PHN) is accountable for assuring a high standard of quality and coordination of care, in support of HealthONE value based care (VBC) programs with payers in the market. The PHN has a defined role, working in an advisory/coaching capacity to support the CCP practices and assist in creating and monitoring standards of care and optimization of performance from value based contracts. The PHN utilizes an evidence-based approach to facilitate QI methodology and tools within practices, including baseline assessments, reporting measures and facilitating meetings with practice specific QI teams working towards achieving measurable outcomes.

DUTIES INCLUDE BUT NOT LIMITED TO:
Obtains and analyzes quality metrics and reports for care opportunities, supports practice implementation of care coordination and care management, identifies action plans for providers to implement to improve cost, quality and the patient experience and participates in design, development and implementation of community learning forums.

The requirements for the PPCN include, but are not limited to:
B.A./B.S. in health care related field coupled with 5 years of experience in primary care, ambulatory care or other related field. Commensurate experience in healthcare, specifically VBC programs, may take the place of the B.A./B.S. requirement.
Preference will be given for a Master’s degree, and/or a strong background in one or more of the following: VBC programs, quality or process improvement, data analysis, healthcare recognition/accreditation, change management and personnel management
Excellent computer skills required, particularly related to Microsoft applications Word, Access, Visio, Excel, PowerPoint and Outlook.
Commitment to collaboration, professionalism, and effective communication in all interactions with physicians, HealthONE Physician Services employees, patients, caregivers and payers.
