Work Schedule: PRN/As Needed Schedule
As an RN Case Manager for Inpatient Rehabilitation at HCA Houston Healthcare North Cypress in Cypress, Texas, you will drive patient-centered care and complex discharge planning within our state-of-the-art, 23-bed acute inpatient rehabilitation unit. Located in the North Patient Tower, this dedicated unit supports patients recovering from major surgeries, traumatic injuries, serious illnesses, amputations, and complex orthopedic, pulmonary, cardiovascular, or neurological conditions. In this role, you will collaborate with an expert multidisciplinary team leveraging specialized features—including a training kitchen, spacious private suites, and a therapy gym equipped with advanced rehabilitation technology—to facilitate seamless transitions and optimize long-term patient recovery.
Job Summary and Qualifications
As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.
What You Will Do In Your Role:
- Manages rehabilitative care to ensure optimum patient outcomes.
- Delivers high-quality, patient-centered care.
- Provides, coordinates, and directs care specific to each patient's needs.
- Collaborates with directors on departmental functions.
- Coordinates efforts across the entire rehabilitation team.
What qualifications you will need:
- Graduate of an Accredited School of Nursing is required. BSN is preferred.
- Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date).
- Current BLS Certification are required.
- Certification in Case Management preferred.
- Certified Rehab Registered Nurse (CRRN) preferred.
- 1-year experience in acute care or inpatient rehab as a licensed RN required
