Licensed Practical Nurse (LPN) – Transition Team

About the Transition Team
Our Transition Team is a supplemental care unit of highly skilled nurses whose primary focus is to quickly and efficiently support patients transitioning home while permanent nursing staff are actively recruited. This team is deployed immediately after a patient referral is received and the patient is admitted to Maxim.
Transition Team nurses provide nursing care for all authorized hours in the home during a provisional period of 1–90 days. During this time, our office team recruits permanent caregivers. Once identified, those caregivers are given the opportunity to train and shadow alongside Transition Team nurses in the home before assuming independent care. After the transition is complete, Transition Team nurses move on to support the next new admission—providing variety, consistency, and meaningful impact in patient care.

Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care
- Initiate and update plans of care as directed; request provider revisions as patient needs change
- Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals
- Respond to changes in patient condition and report to physicians or supervisors as appropriate
- Administer prescribed medications and treatments and document patient response
- Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning
Requirements
- Current Licensed Practical Nurse (LPN) license in the state of practice
- Current Health Certificate (per contract or state regulation)
- Current PPD or Chest X-Ray
- Current BLS certification
- One year of LPN experience preferred
