Responsibilities
- Provide skilled nursing care in the patient's home
- Conduct comprehensive patient assessments and ongoing monitoring
- Administer medications, treatments, and physician-ordered interventions
- Implement and update individualized care plans
- Monitor and document patient progress and changes in condition
- Collaborate with physicians, caregivers, and the clinical care team
- Provide education and support to patients and family members
- Maintain accurate and timely clinical documentation
- Respond appropriately to patient needs and changes in health status
Qualifications
- Current Pennsylvania Registered Nurse (RN) License in good standing
- Current BLS/CPR Certification
- Minimum one year of nursing experience preferred
- Homecare, skilled nursing, rehabilitation, pediatric, critical care, or trach/vent experience is a plus
- Strong assessment, communication, and clinical decision-making skills
- Ability to work independently in a home setting
- Reliable transportation and dependable attendance
