Job Summary and Qualifications
The individual is responsible for facilitating admissions of HCAT patients from HealthONE emergency departments to behavioral health hospital services via management of phone triage. He/she will work closely with all healthcare providers to facilitate and coordinate efficient, effective, quality healthcare. The individual must demonstrate knowledge of the principles of growth and development across the life span; possess the ability to assess data reflective of the patient’s clinical status and interpret the appropriate information needed to identify each patient’s requirements related to his or her age-specific needs. He/she will utilize various computer technologies to ensure communication between HCAT and emergency departments regarding the flow of mental health patients. He/she will implement and comply with all the policies, procedures and standards of the unit, department and the institution. The HCAT Placement specialist must be available for 12-hour shifts with rotating weekends.
What you will do in this role:
The HCAT Placement Specialist is responsible for providing services to patients who have complex psychosocial and discharge planning needs. In addition, the HCAT Placement specialist community assistance, and under the guidance of the HCAT Clinician, coordinates and facilitates the development of a Discharge Plan of Care for at -risk patient population with discharge needs. The HCAT Placement specialist also functions as a member of the Interdisciplinary Patient Care Team, who communicates and receives referrals from other patient care providers to help meet the needs of the patient and family for a safe discharge.
What qualifications you will need:
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
- High School Graduate / GED
- Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
